Promoting Quality of Life initiatives (A Program of Manna Café)
Championing Older Adults’ Comprehensive Health (C.O.A.C.H) addresses health challenges while promoting quality of life initiatives primarily for low-income residents of Montgomery and Stewart Counties.
Program Overview: C.O.A.C.H. offers a multifaceted approach to health and wellness promotion while targeting social and economic factors and clinical care to drive health outcomes. It consists of the following program components:
Food and Wellness Education: Nutritious meals and grocery provisions paired with health and wellness education benefiting low-income, food insecure residents of Montgomery and Stewart Counties.
Health Services: Free health clinics for low-income community members are offered twice monthly by a Licensed Physician at The Refuge Community Center. Clinical service provisions can include health screenings; basic wellness check-ups; monitoring chronic illnesses; medication prescriptions; physicals; referrals; and consultations.
Casework Assistance: Free access to consultation and support services specific to clients’ unique situational needs while collaborating with other local agencies to receive and direct referrals and effectively utilize community resources. Promotes programs that provide hope for economically insecure older adults, such as NCOA’s Center for Benefits Access, which helps enroll older adults with limited means into benefits programs for which they are eligible, so they can remain healthy, secure, and independent and pay for health care, medicine, food, utilities, and more.
Civic Engagement: Offers weekly opportunities for older adults to experience a sense of community, connection, and purpose through volunteering.
Target Population: The program is intended to support low-income older adults (ages 50+) and their families residing in Montgomery and Stewart Counties.

Manna Café Ministries is a Partner Agency of Second Harvest Food Bank of Middle Tennessee.
Second Harvest is a private, not-for-profit, tax-exempt organization with a mission to feed hungry people and work to solve hunger issues in our community. Second Harvest works with a network of approximately 500 Partner Agencies like Manna Café Ministries to distribute food to those in need throughout 46 counties in Middle and West Tennessee.
